A photo's size in centimetres depends on the DPI it's printed at. At 300 DPI, the usual print resolution, 3.5 cm is 413 pixels and 4.5 cm is 531 pixels. We resize to that and write 300 DPI into the file, so it prints at exactly 3.5 × 4.5 cm.
Stand against a plain white or light wall with even light on your face, no shadows behind you. Look straight at the camera with your face and both ears visible. Don't wear a cap or dark glasses unless your form allows it.
The photo is cropped to the 3.5 : 4.5 shape from the centre, so keep your face in the middle of the frame with some space above your head.
The default limit is 50 KB, common on government forms. Change it to match your notification. Some exams also need a minimum, such as 20 KB; set that too.
