Required: Passport-size photograph, Signature, Class X or Equivalent Certificate/Marks-sheet. Only if they apply to you: PwD/PwBD Certificate, Identity Proof, Disability/UDID certificate. The application form also takes a live photo with your webcam or phone camera.

Yes. Live photograph is taken by the application form itself, with your webcam or phone camera, so there's no file to prepare for it. You still upload a photo file as well; prepare it on this page. Use a plain, light background, even light, and look straight at the camera.

As PDF files of 50–300 KB. Drop a PDF, or photos of each page, onto the certificate's card: they become one A4 PDF inside the allowed size, and you can download it straight away.

We first raise the quality and resolution within the allowed limits. If it's still under the minimum, we add standard metadata to reach it. The picture doesn't change, and the card tells you when this happened.
